Have you had an appointment at Medpeds in 2010 yet?
It’s a new year! Therefore we need all patients to sign new forms along with re-presenting their photo id and insurance cards at their first appointment in 2010. We realize that you have filled out these forms as well as provided your photo id & insurance cards in the past; however, in an effort to ensure your chart is updated, accurate and meets Medpeds guidelines, along with many insurance guidelines, we need proof of re-verification of all patient information and disclosure forms to be dated & signed in the current year (i.e. 2010).
Please be prepared to present ALL of the following items when you arrive at your first 2010 appointment:
1. COMPLETED PAPERWORK
(If you don’t have your completed paperwork with you, we will need you to arrive at least 30 minutes BEFORE your appointment time.)
- Patient Updated Information 2010
- Consent 2010
- Medical History 2010
- Medical Records Release 2010
- Referral Policy 2010
- Our Privacy Policy
2. PHOTO ID and ALL INSURANCE CARDS
3. NEW YEAR = NEW DEDUCTIBLE
Deductible (Medicare not applicable) for the 2010 New Year and/or Co-Pay (per your insurance).
Remember:
Check In Time is 15 minutes prior to your appointment. If you are unable to keep your appointment, please notify our office at least 24 hours in advance in order to avoid a no show fee.
